Use this site to access and download information on several of our policies, forms, & waivers. The list below provides some of the most relevant forms that each REC may need for daily operations. The list is sorted alphabetically. Click on the title of each form to download the blank form or to forward you to another site. Once the form is downloaded, you are able fill out the form and sign.

Equipment Use, Storage and Maintenance Agreement | When a PI wants to store their equipment at a REC for use on their project. Extends the agreement to include approved users who have completed training and accepted terms. Lists supporting documents, such as training records, policies, and other relevant materials. |

| Request for Assistance to Leadership (Director of REC System) | Use this form to request items/services for your REC from the REC System Director. These requests are separate from large REC based projects. |
| Research Advisory Committees (RACs) | Describes the policy and process whereby academic research and extension project proposals are submitted for consideration, recommendation and approval / disapproval. |

Utilization of space at UC ANR's RECs for UC Academics or Staff not assigned to a REC. For office and storage space use record management. Use the Facility Use Fee Template to determine your building maintenance costs to then determine a rental fee. Use the ANR Building Summary sheet to find out square footage of your buildings. | |
