All EFNEP participants should provide their enrollment information on the About Me form. This form may be printed in-house or ordered through the State Office. It is also available electronically in Qualtrics.  The following list provides PDFs of the About Me form in the language indicated.

Activity Waiver

This waiver is encouraged for participants who would like to participate in cooking and physical activity in class.
 

Photo Release

This waiver is required for participants who consent to have their photos taken. This should also be used for video and audio recordings.

  EFNEP Favorite Recipes

A small sampling of favorite foods shared during EFNEP classes over the past 50 years. Recipes come from many of the universities operating EFNEP in each state, the   District of Columbia and in six U.S. territories. This collection may remind you of a family cookbook, as several recipes include a comment or story shared by a participant or a peer educator.

  Preparedness & Resiliency Cookbook

Provided by a national EFNEP workgroup.  A guide for making good food choices that are healthy, tasty, quick, and safe, to promote resiliency during hardship or natural disasters. This guide provides 21 meals and 5 snacks to feed a family of 4 for 7 days. Most of the ingredients in this recipe book are based on canned, boxed, or
bottled non-perishable food items, except some fresh fruits and vegetables.
